The OSU Award for Outstanding Service to Persons with Disabilities

Introduction: The University Advisory Committee, Commission on Status of Individuals with Disabilities (COSID), has been annually honoring an individual who advocate the inclusion, accommodation, and respect of persons with disabilities with an award. For the past eighteen years this award has been given to recognize individuals who foster and promote persons with disabilities to enjoy full participation in all aspects of the campus community.

Title: OSU Award for Outstanding Service to Persons with Disabilities

Eligibility: Any OSU student, staff or faculty

Nominations: Nominations for the Outstanding Service to Persons with Disabilities Award are invited from faculty, staff, and students. The award is to recognize an individual who has provided outstanding service to those with disabilities. Nominations from individuals with first-hand knowledge or experience with nominee’s efforts, in particular, are most welcome.

Award Selection Committee: An award selection committee comprised of members affiliated with the COSID will evaluate the nominees.

Nomination Closing Date: May 15, 2015

Nomination Procedure: Send completed nomination form, letter of nomination (include details of activities that justify nomination) and other supporting documentation, i.e., letters of support.
