2014 Nomination Form

The OSU Academic Advising Award recognizes undergraduate academic advising by professional faculty members and acknowledges advising as a profession making a pivotal contribution to the OSU community.

Nomination packet general instructions appear at the end of this site. It is necessary to follow the instructions completely or the nomination will not be considered.

Do not begin entering information on this electronic form unless you are ready to submit the nomination - the form CANNOT be saved and finished at a later time. Please send a hard copy of the original nomination materials to the Faculty Senate Office (instructions below) when the nomination is submitted electronically. Nominations will not be considered unless both the electronic version and hard copy original have been received by the Faculty Senate Office.

Please upload a file for the items below which explains to what extent the nominee fulfills each of the selection criteria for the award.

Nominations will be evaluated on the evidence of effective advising qualities and practices that distinguish the nominee as an outstanding student advisor. Such evidence will include the following:

  • Attitude: Evidence of demonstrating a professional attitude toward advisees through interpersonal skills and accessibility.

  • Influence: Evidence of positively influencing and monitoring student academic success and development, as well as mentoring colleagues:

  • Contribution to the University community: Evidence of promoting new and innovative programs that promote student success and development:

  • Involvement: Evidence of demonstrated leadership, present or past, in a professional organization related to student development:

  • Professional Development: Evidence of pursuing professional development, such as attendance or presentation at seminars and conferences, at OSU or elsewhere:



  • Nominee's Curriculum Vitae:

  • Nominee's position description indicating breakdown of FTE allocation:

Three letters of support: one letter of support shall be from a current or former student, one shall be from a current or emeritus faculty member and the third may be from a student, faculty member, alumni or other individual who can comment on the nominee's qualifications.The nominator is not eligible to submit a letter of support. The letters of support each shall not exceed three single-sided pages using at least 12 font and 1" margins. Signatures on the letters and supporting documents must be included, but may be electronic.

Additional information submitted with the nomination will not be considered; please include only requested information and refer to the guidelines when organizing nomination packets.

The requested electronic information and one original of each nomination for for the OSU Academic Advising Award must be received by February 11, 2014. It is necessary to follow the instructions completely or the nomination will not be considered. Send the original nomination to: Faculty Recognition and Awards Committee, c/o Faculty Senate Office, 107 Gilkey Hall, Corvallis OR 97331-6203.

The original copy of the nomination form must be submitted to the Faculty Senate Office upon submission of the electronic copy; paper clip the original only – do not staple nor place it in a folder or binder. The nomination packet for the successful recipient becomes University property and will not be returned to the nominator.
