Data Systems Access

Access to any of the Banner, Banner Web, and Data Warehouse systems is dependant upon having both an authorized, completed, and approved Banner and Data Warehouse Request for Access form, and a signed Confidentiality and Policies statement on file. Most access also requires attendance and completion of specific workshops which are identified by a note in the workshop descriptions in this document. Please also note that Data Warehouse access is based on having similar access in Banner.

Because it can take several days to get all the approvals and processes done to create an account, we ask that you send in your completed Banner and Data Warehouse Request for Access form and signed Confidentiality and Policies statement as early as possible. You may campus mail the forms to "Banner and DW Access, 206 MCC" or hand carry the two forms to Milne 206.

Both forms and instructions are available at the web site: http://oregonstate.edu/dept/computing/banner/access.html.

If you have any questions about the status of your access to Banner, Banner Web, or the Data Warehouse,
please call: Carla Simonson at (541) 737-2671, or Dawn Belveal at (541) 737-2694.

Disability Accommodations

Requests for accommodations related to disability need to be made to Ron Stewart at 737-7307 at least five working days prior to the workshop. A "Request for Accommodation" form can be found at the OSU Technology Access Program web site.

Cancellation Policy
