Forms

The CCE, in partnership with the Center for Fraternity and Sorority Life, is excited to release the 2012 Community Values Award, which honors the Greek organization with the highest average community service hours per member. We challenge you to become engaged, affect real change in your community, and earn local and national recognition as the Greek chapter most dedicated to service.

Forms are due the first day of Dead Week each quarter to be totaled at the end of the calendar year.  The award runs via the calendar, not academic, year and therefore includes winter, spring and fall quarters (summer not included).

In order to record the hours of service you or your group completes, first you'll need to fill out a short form. Then, after you complete your service, deliver it to the Center for Civic Engagement in Snell 158 . You can pick up these forms at the CCE or you can print them here.

If you are conducting a one time service project individually, you will need a One Time Service Sign In Sheet.

If your group is conducting a project together, your group will need a Group Service Sign In Sheet.

If you are participating in ongoing service individually or with a group, you will need an Ongoing Service Sign In Sheet.

Once you have completed a Service Project you can fill out a Volunteer Experience Survey.
