The agenda for the October Faculty Senate meeting will include the reports and other items of business listed below.

   1. Approval of June 2011 Minutes
   
   
   2. Category I Proposals
      Mike Bailey, Curriculum Council Chair, will present for approval the following Category I proposals:
      
      • Merger of the Department of Geosciences with the College of Oceanic and Atmospheric Sciences: Creating the College of Earth, Ocean, and Atmospheric Sciences
      • New Degree Program – Earth Sciences (BS)
      • New Degree Program – PhD in Public Policy
      • New Degree Program – B.A., B.S., B.F.A in Digital Communication Arts


2. SPECIAL REPORTS
   
   
   1. Task Force on Shared Governance
      Lynda Ciuffetti and Becky Warner, task force co-chairs, will present the final report for endorsement by the Faculty Senate. Revisions to the report appear highlighted in yellow.


3. REPORT FROM AND DIALOG WITH THE FACULTY SENATE PRESIDENT

4. STATE OF THE UNIVERSITY ADDRESS
   
   President Ed Ray will deliver his State of the University Address titled "Next Steps”


5. INFORMATION ITEMS
   
   
   1. Mandatory Open Enrollment for 2012 PEBB Health Insurance
      

      PEBB Health Insurance Open Enrollment is October 15 – November 15, 2011. Open Enrollment is MANDATORY for selecting a 2012 medical and dental plan OR to continue to Opt-Out of the medical plans.

      If you do not enroll or opt-out of a 2012 medical and dental plan during Open Enrollment:

      • Employees with dependents – Your dependents will lose coverage and you will be enrolled in the PEBB Statewide Non-HEM plan and the ODS Traditional dental plan effective January 1, 2012. You will also be charged the $25 per month Tobacco Surcharge.
      • Employees with employee only coverage – Your coverage will be changed to the PEBB Statewide Non-HEM plan and the ODS Traditional dental plan effective January 1, 2012. You will also be charged the $25 per month Tobacco Surcharge.

   2. Interinstitutional Faculty Senate Recap
      

      A written report recapping the October 7 and 8 IFS meeting will be prepared by Starr McMullen, IFS Senior Senator, and distributed separately from the agenda.

   3. Faculty Senate Calendar
      

      Please reserve the following dates for Faculty Senate meetings during academic year 2011-12; check your monthly agenda to determine the location. All meetings are scheduled to begin at 3:00 PM:

      • 2011 - November 10, December 8
      • 2012 – January 12, February 9, March 8, April 12, May 10, June 7

   4. Faculty Senate Elections – Faculty with Split Appointments
      

      According to the Faculty Senate Bylaws, faculty who have split appointments between two apportionment units may choose with which unit they wish to be apportioned for Senate purposes. For example, if a faculty member has a split appointment between Agricultural Sciences and Science, they may choose one or the other (FTE will not be split among units). This choice will determine Senate representation. Faculty not choosing where they wish to be apportioned, will be placed in an apportionment unit at the discretion of the Faculty Senate Office. Please contact the Faculty Senate Office (facullty.senate@oregonstate.edu) prior to October 20 to indicate with which unit you wish to be apportioned. When responding, please provide your name, the units you are split between and the one with which you prefer to be apportioned.

   5. Faculty on 1039-Hour Appointments
      

      Retired Faculty Senators whose terms extend beyond December 31, but whose 1039-hour appointments terminate fall term, are no longer eligible to serve as a Senator. If you fall into this category, please contact the Faculty Senate Office at faculty.senate@oregonstate.edu so a replacement can be identified to complete the remainder of your term.

      Faculty whose Senate committee/council terms extend beyond December 31, but whose 1039-hour appointments terminate fall term, are no longer eligible to continue serving. If you are not filling a specified emeritus/retired position on the committee/council on which you serve, or if you are unsure, please contact the Faculty Senate Office so a determination may be made and/or a replacement identified.

   6. Category I Approval
      

      The campus community should take appropriate action to update their records to reflect approval of the following OSU Category I proposals:

      • Extend (from OSU-main to OSU-Cascades): BS in Exercise and Sport Science: Applied Exercise and Sport Science Option

   7. University Day
      

      The 2011 University Day program, including Dr. Kevin Fitzgerald’s keynote address titled ‘A Morning of Laughter and Learning,’ is streamed online at: http://oregonstate.edu/events/universityday/2011-recap-video

6. NEW BUSINESS
