Invention Disclosure Forms, Confidentiality, and Material Transfer Agreements

 

Disclosure Forms

Four distinct disclosure forms are used by the Office of Technology Transfer (OTT) to evaluate OSU innovations. Once one form is completed and submitted, the OTT will set up a file and begin the process. Please choose the most appropriate form for your innovation by reviewing them below.

Once a disclosure form is received by the OTT, each inventor/contributor will receive an email confirmation, which includes an OSU docket number and the contact information of the licensing professional assigned to your innovation.

Invention Disclosure Form

The most commonly submitted form for most innovations. Copyright Disclosure Form

Software and Digital Media Disclosure Form

Plant Variety Protection Disclosure Form

Confidentiality and Material Transfer Agreements

OSU employees do not have signature authority to sign confidentiality agreements (such as CDA, NDA, CITR) or material transfer agreements (MTA) on behalf of the university. The OTT has the appropriate authority. Our licensing professionals are experienced in the efficient negotiation and execution of these agreements.
