Project Revision

 

From time to time, an approved project may need to be revised. Before any change may be applied to a project that has been approved, the proposed change must be reviewed and approved by the IRB. The exception to this is when immediate action is taken when necessary to eliminate apparent hazards to subjects, but this revision to the approved project must be reported immediately to the IRB (submit an Exception/Deviation form in addition to the Project Revision form; both are available on the OSU IRB website).

To revise an existing project, submit a completed Project Revision form, available on the OSU IRB website. Include any new or revised supplementary documents (e.g., letters, surveys) and a copy of the protocol with proposed modifications incorporated, with any proposed changes highlighted.

Revisions to existing approvals do not extend the approval period of the project. Each project will be required to go through a continuing review no less than once each year based on the date of original approval.
