IACUC Forms

Please remember that the IACUC is comprised of your colleagues and peers. In addition to their regular job duties, members serve as volunteers on this federally required institutional committee. Although they will try to respond as quickly as possible, the review process likely requires 8-10 weeks, or longer depending on the complexity of the review, on the need for a full committee review, or if modifications to the application are required. Please plan appropriately.
- Animal Care and Use Proposal (ACUP) Form 2013
This form is for new or continuing projects. It must be completed and approved by the IACUC, prior to initiating vertebrate animal projects.

Recommended as a reference when completing or submitting with the new ACUP form
- Annual Renewal and Final Report Form (PDF)
Required every year for renewal of approved projects and when an ACUP is closed/expired.
- Animal Care and Use Amendment Form
Required for any changes to previously approved ACUPs.
- Animal Care and Use Change in Personnel Form
Use this form when the only change to the IACUC approved project is personnel related.
- Animal Care and Use Incident Report Form
Submitted for any unexpected negative results.
