What types of activities/interests do I have to disclose?
The questions on the form relate to your financial interests outside of the University, including equity interests in public and non-public entities, consulting, outside employment and compensated service on advisory or scientific boards. If you identify that you hold a Significant Financial Interest in an outside entity, additional questions will ask additional information about each interest that you have.
