What is a disclosure form ("declaration")?
A disclosure form, or “declaration,” is a self-reporting form completed by investigators to disclose to the University their outside activities and financial interests. The form is a tool to gather information that will allow the COI Officer to determine whether a conflict of interest or the appearance of a conflict of interest exists. Disclosing activities or interests on the form does not mean that those disclosures are conflicts of interest.
