Use this form to request updates or make clarifications to the Degree Audit System. This information will be sent to the Degree Audit System Analyst. This is meant as a supplement to the Catagory II proposals, to capture information not submitted via the "official" program change channels. While this will not cover all possibilities, it will inform us that there is something to be updated and allow us to do a little research before contacting you with questions we may have. Note: This form is for overall requirements updates only. Individual substitutions and adjustments should be done via the Substitutions and Waivers Form (under construction)
