Online Reporting
The webforms below allow students on an F-1 or J-1 visa to report certain changes to International Student Advising and Services (ISAS). Your information will be emailed to an international student advisor, who will update your information in SEVIS, meeting your reporting requirement.
Change of Address
Students on an F-1 or J-1 visa must report a change of residential address to ISFS within 10 days of moving. If you are a currently enrolled student, you can login to make these changes through MyOSU or Student Online Services.
If you have already graduated and are on Optional Practical Training or Academic Training, you will need the following information to report this change:
- Full legal name
- OSU ID number
- New residential address

Employment Update
Students on an F-1 visa must report a change in employment status while on Optional Practical Training, which includes beginning, ending, or changing employment. To report a change in employment status, you will need the following information:
- Full legal name
- OSU ID number
- Employer (company) name
- Employer address
- Start and end dates of employment

17-Month OPT STEM Validation Report
Students on an F-1 visa who have been approved for a 17-month extension of their OPT employment benefit must submit a validation report to ISFS every six months, beginning six months after their extension start date. To submit a validation report, you will need the following information:
- Full legal name
- OSU ID number
- SEVIS ID number (begins with "N" and is found in the upper right hand corner of your I-20)
- Current residential address
- Employer (company) name
- Employer address
