Online Reporting
The webforms below allow students on an F-1 or J-1 visa to report certain changes to International Student & Faculty Services (ISFS). Your information will be emailed to an international student advisor, who will update your information in SEVIS, meeting your reporting requirement.
Change of Address
Students on an F-1 or J-1 visa must report a change of residential address to ISFS within 10 days of moving. You will need the following information to report this change:
- Full legal name
- OSU ID number
- New residential address

Employment Update
Students on an F-1 visa must report a change in employment status while on Optional Practical Training, which includes beginning, ending, or changing employment. To report a change in employment status, you will need the following information:
- Full legal name
- OSU ID number
- Employer (company) name
- Employer address
- Start and end dates of employment

17-Month OPT STEM Validation Report
Students on an F-1 visa who have been approved for a 17-month extension of their OPT employment benefit must submit a validation report to ISFS every six months, beginning six months after their extension start date. To submit a validation report, you will need the following information:
- Full legal name
- OSU ID number
- SEVIS ID number (begins with "N" and is found in the upper right hand corner of your I-20)
- Current residential address
- Employer (company) name
- Employer address
