Forms


Educational Activities Fund
As recognized student organizations, Greek groups are eligible for educational activity fee money for campus programming. Forms may be picked up in the ASOSU offices in Snell Hall.

Event Registration
Register all your chapter's social and philanthropic events online. Registration must be received at least two full weeks prior to the event. All functions with alcohol must be registered. Open events, such as fundraisers and philanthropic events, must also be registered. Online registration.

Fire Drill Reports
Register your chapter's fire drill online. Chapters must report two fire drills per term following the terms by the Office of Greek Life. Please see the Fire Drill Section under Risk Management. Online registration.

Grade Release Forms
In order for individual student grades to be released to the President of the chapter, Grade Release Forms must be completed and signed by those students who agree to the release of their grades to the Chapter President. Those forms are distributed by the Office of Greek Life midway through the term and are due by the last day of each Fall, Winter and Spring term.

Permanent Membership Cards
A Permanent Membership card on every member of the Greek Community is maintained in the Office of Greek Life. A completed card on each new pledge to a fraternity or sorority must be turned into the Office of Greek Life by the chapter within 48 hours of the student's acceptance of a bid from a chapter. The Membership Status Update process of all members of every chapter is completed by the Chapter President no later than the end of Dead Week each Fall, Winter and Spring term.

Guidelines for Responsible Use of Alcohol
Return a completed copy to the Office of Greek Life at least two weeks before any function with alcohol. Download and print this form (pdf).

Philanthropy Financial Report
Return a completed copy to the Office of Greek Life within 10 working days after the event. Download and print this form (word).
