Exam - Sealed Source Training
Instructions:
Before beginning the exam, make sure you ask any questions you have about this training. You can either ask your Program Director, or contact Radiation Safety:

Follow these steps to complete the exam:
- Print out the exam (Adobe pdf link listed below)
- Answer all the questions
- Have the Program Director sign as "examiner"; this indicates that the Program Director has proctored the exam and is satisfied that the examinee understands all the information on the exam.
- New users must also submit the Orientation Acknowledgement at the end of the exam (new users only). For refresher training, only the exam needs to be submitted.
- Sign as "examinee", and mail or fax exam and the completed "Orientation Acknowledgement" to:
