Lab Hazard Sign Request

A hazard sign of the type depicted below will be placed at every main entrance to a lab room or lab complex.  The signs are produced by EH&S, but it is the responsibility of laboratory personnel to provide information to keep the sign up-to-date. 

Use the Lab Hazard Sign Information Form to request a new or updated Lab Hazard Sign.
