If you would like to order or receive laboratory animals from an approved Commercial Vendor, submit the completed form via email to: larc@oregonstate.edu 

• Animal Transfer Form--Internal

If you would like to transfer animals from one ACUP # to another ACUP # or transfer to one scientist to another scientist at OSU, submit the completed form via email to: larc@oregonstate.edu 

• Animal Import Transfer Form from Non-Commerical Institutions/Organizations to OSU

If you would like to order animals from a Non-Commercial Institution/Organizations to OSU, submit the completed form via email to: larc@oregonstate.edu.  Allow 4 weeks prior to the desired arrival date.
