Instructions for using the electronic application form

1. Type the information into the form below and choose "Send the Application".
Your application will be received electronically and a place at the ELI will be reserved for you.

2. Mail the following items:

A. a $75.00 non-refundable application fee (a check in US dollars) if you did not include a credit card number with the online form. If you paid by credit card, you do not need to pay again.

B. financial support documents: letter of financial guarantee and affadavit of support, bank statement, and (if applicable) scholarship award letter. You can print an affadavit of support to send in by mail.
These documents must be in English and less than a month old. You need to show a minimum of $18,375 available for your first 9 months (3 terms) in the U.S.

C. a photocopy of the title page of your passport.

D. If you are under 20 years of age, you will need to include an official transcript in Englishfrom the academic institution you most recently attended.

E. Your name, mailing address, e-mail and fax if available, and date of birth as shown on your online form. This will help us make sure we put the online application with the correct supporting documents.

For optional Express Mail Service, add an extra $30. Your admission letter and I-20 will be sent by express courier.

Your electronic reservation is valid for 30 days only. If the application fee and other documents are not received within thirty days, your reservation will be canceled.

Remember, we can begin working on your application when it arrives and if you fax us the supporting documents, but your I-20 must wait until we receive the original, signed Affadavit of Support, bank statement, scholarship award letter (if applicable), copy of the title page of your passport, and (if you are under 20 years of age) transcripts. We can send your I-20 only after we receive all the original supporting documents in English.

Don't forget to mail your:
  1. original (not a photocopy) financial support documents -- in English

  2. photocopy of the title page of your passport - this is the page with your name and passport number on it

  3. If you are under 20 years of age, an official transcript in English from the academic institution you most recently attended.

  4. and

  5. a $75 non-refundable application fee in the form of:
  • a check in US dollars to the English Language Institute

    or

  • direct payment by credit card (Visa or MasterCard only). Include a photocopy of your credit card with your original documents.

Certification and Agreement

To apply to the ELI on your computer, please send the completed application form with your (optional) credit card payment now. When you click on "Yes, I agree" below, you agree to the following statement:

I hereby certify that all the information on this application form is true and accurate and that the stated funds are available for my educational expenses at the English Language Institute (ELI). I will notify the ELI immediately of any changes in my financial circumstances. I understand that the submission of inaccurate information can be considered sufficient cause for rejecting my application or terminating enrollment at the ELI.
