Supplemental Documentation
The organizational chart is required for all PDs. Additional documents may be required, depending on the type of action submitted. The screen below from the "Online PD/Recruitment System" indicates the types of documents that may be included.

Organizational Chart
Important!
A current organization chart is required to be attached to each PD.
It is required that the organization chart be included when submitting a PD. You may submit an existing organization chart if
it is accurate, up-to-date and provides the required information. The chart should be sent in Microsoft Word or .pdf format. The organization chart must:
- Highlight or clearly designate the subject position.
- Identify at least two levels of supervision above the subject position.
- Identify all other positions directly reporting to the same supervisor.
- If this is a "leadwork" position, identify the positions for which this position provides
leadwork.
- Identify all positions by classification title and position number.

Requisition Form
If you are filling a position, this section will need to be completed; however, it is not part of the PD format.
A PD exists for every position whether filled or vacant. Determining the correct classification, proper budgeting and effective
recruitment requires an accurate PD. Revise the
PD whenever duties/ responsibilities change significantly, or when other changes such as position number or geographic location occur.
