PEBB Health Insurance Open Enrollment October 15 – November 15, 2011
Open Enrollment is MANDATORY. for selecting a 2012 medical and dental plan OR to continue to Opt-Out of the medical plans.
This is also your once-a-year opportunity to make changes to your benefit packages without having a qualifying family status change (marriage, birth, divorce, loss/gain of other insurance, etc.) and to re-enroll in the Flexible Spending Accounts (FSAi) for 2012.
If you do not enroll or opt-out of a 2012 medical and dental plan during Open Enrollment:
- Employees with dependents
- PEBB will enroll you in the Employee-ONLY tier of the PEBB Statewide medical & ODS Traditional dental plans effective January 1, 2012
- Your dependents will lose medical and dental coverage effective January 1, 2012
- You will be charged a $25 monthly Tobacco Surcharge
- You will be charged a $20 monthly HEM Surcharge
- Employees with employee only coverage
- PEBB will enroll you in the Employee-ONLY tier of the PEBB Statewide medical & ODS Traditional dental plans effective January 1, 2012
- You will be charged a $25 monthly Tobacco Surcharge
- You will be charged a $20 monthly HEM Surcharge
If you currently decline benefits, you are not required to participate in Open Enrollment and are not subject to the HEM or surcharges.

HOW To Enroll or Make Benefit Plan Changes
- Online Enrollment on or after October 15, 2011 through midnight November 15, 2011: https://pebb.benefits.oregon.gov/members/

WHEN are Changes Effective – Changes made during Open Enrollment are effective January 1, 2012, except for policies and/or amounts that require the approval of underwriters (life/ long-term care).
VERIFY that Changes Have Been Processed – Review your Earnings Statement to confirm that your enrollments and deductions are correct.
- December 2011 Earnings Statement will reflect changes to Medical, Dental, Life, Accidental Death, Short-term Disability, Long-term Disability, and Long-term Care.*
- January 2012 Earnings Statement will reflect enrollment in the Flexible Spending Account(s) for 2012.
*For policies that require approval of an underwriter the change will be on the Earnings Statement the month following approval (life/long-term care).
