Check YES if driving a University owned (non-motor pool) or personal vehicle on behalf of the University is an essential function of the position and the incumbent has duties requiring driving that cannot be performed by others. Include positions where the incumbent may have the need or opportunity to transport passengers at any time while conducting university business.

If a motor vehicle history check is required for a position, it becomes a minimum qualification for the position. List the requirement in the Minimum/Required Qualification section of the Position Details page of your action as follows:

This position requires driving a University vehicle or a personal vehicle on behalf of the University; therefore, the incumbent must successfully complete a Motor Vehicle History Check, possess and maintain a current, valid driver's license in their state of residence, be determined to be position qualified and self-report convictions (as per Voluntary and Compulsory Driver Standards OAR 125-155-0200) as per OAR 576-056-0000 et seq.

Instructions for submitting a Motor Vehicle Check:

The incumbent is required to possess and maintain a valid driver's license in their state of residence. Please obtain your non-employment driving record/history from the issuing state's DMV for the past 24 months and either submit an electronic copy to DPSCHC@oregonstate.edu, or fax a copy to 541.737.0468 Attn: Michele Spaulding (most DMV offices will fax directly to me if requested) or submit a hard copy to:

If satisfactory Motor Vehicle Check results have not been reported prior to the offer, the offer must be contingent upon satisfactory Motor Vehicle Check results. The applicant CANNOT BEGIN WORKING IN THE POSITION UNTIL SATISFACTORY RESULTS HAVE BEEN REPORTED. This applies to ALL positions, even temporary positions.

The supervisor will be notified by OHR when the check is complete.
